Social Worker Neighbourhoods - Adults (East & West)
Starting salary £37,280 rising to £41,771 per annum. Hybrid working options. 30 days annual leave plus bank holidays, with the option to buy additional leave. Grade 10 post in Somerset Council.
What will I be doing?
Social Workers in the neighbourhood teams work closely with health and community colleagues to understand community resources, facilitate independence, maintain people’s ability to live in their own communities, and work with a strengths-based approach. You will:
* Work in the office base for a minimum of 2 days per week, with the rest of the week supported by technology in a hybrid model.
* Utilise extensive skills and experience to support and empower residents of Somerset to remain independent longer in their communities.
* Tailor solutions around residents’ needs and provide continuity of support and intervention.
* Promote independence in all team activities.
* Work alongside Health and Social Care colleagues as part of the neighbourhood PCNs.
* Liaise with key partners to improve provision of services, training, and recruitment for the team.
* Be involved in developing, shaping, and building integrated care local services.
What kind of experience or qualifications do I need?
* Be a qualified Social Worker and be registered with Social Work England.
* Have access to/use of a vehicle (essential for this role).
