We are offering a fantastic opportunity for a Band 6 Band 6 Mental Health Practitioner. (Either a NMC Registered Mental Health Nurse, a HCPC Registered Occupational Therapist or a registered Social Worker) to join our Home Crisis Treatment team based at St Georges Hospital in Stafford.
We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week.
You will be responsible for the care coordination of an identified caseload, and you will also be expected to be available for consultation, support and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate.
As are a supportive team and we encourage you to grow and develop your skills through continuing professional development, training and supervision.
As our team offer a 24-hour service all practitioners are required to work rostered shifts including weekends and nights and you must be able to travel independently over a large geographical area during your shift.
Main duties of the job
As a Mental Health Practitioner within our team, you will be expected to:
Assess mental health crises in individuals referred to the service and supporting the delivery of safe and effective home treatment for an identified client group and in accordance with a wellbeing and recovery action plan
* Participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved.
* Assist in the delivery of a high standard of nursing care to service users referred to the team.
* Implement agreed systems for the monitoring of effectiveness of care.
* Participate in the education and management of junior staff.
