Job Title: Social Worker (Hospital Discharge Team)
Location: Middlesbrough Metropolitan of Lewisham
Rate: £284 per day
An exciting job role has recently become available at the Middlesbrough Metropolitan of Lewisham. They are looking for a new member for their Hospital Discharge Team.
X4 workers required for a 12 month period
2 years post-qualifying experience required. Request to cover case work/reviews. This will involve the ability to undertake face-to-face visits 5 days a week with 2 days a week working from an office setting. Learning Disability Team – adults in transition and over 18 with a learning disability.

Requirements:
4. Assessing Needs: Conduct interviews and assessments to understand the needs of individuals and families.
5. Developing Care Plans: Create and implement care plans to support clients in achieving better outcomes.
6. Advocacy: Advocate for clients’ rights and access to necessary services.
7. Collaboration: Work with other professionals in multidisciplinary teams to provide comprehensive support.
What we are looking for in a Social Worker:
8. Social Work England registration
9. Eligible to work in the UK
10. Hold a full UK licence
11. Post-qualified experience
12. Degree level or equivalent in Social Work
