Service Co-ordinator
An exciting opportunity has become available to work within Care4CE Community Reablement Services. The position is for a permanent Grade 6 Service Co-ordinator 37 hours Monday to Friday 9am – 5pm office-based role at Macclesfield Town Hall. Starting pay £14.82 per hour - £28,598.00 per annum – incremental for 5 years to £16.08 per hour - £31,022 plus an annual pay rise.
About the role
Community Reablement is a short-term service working with our integrated health partners. This role is critical to the effective operation of our service. It is a fast‑paced, high‑volume position responsible for coordinating the full referral pathway and supporting a positive customer journey. The postholder plays an essential part in daily discharge planning by attending discharge meetings and PRE meetings, working closely with our senior practitioners and administrative teams as part of the office duty desk.
The Service Coordinator manages all incoming referrals, working collaboratively with the Prevent, Reduce, Enable Team and hospital discharge teams to ensure timely triage and allocation. The post holder also manages the referrals and allocation of staff for the new Take a Break service providing respite to carers. Key responsibilities include loading and maintaining customer data, allocating and scheduling visits for each individual, and completing the weekly rostering for all Reablement Workers. This ensures safe, efficient deployment of the workforce and supports the delivery of high‑quality, outcome‑focused services.
The post also has a significant partnership function, liaising daily with PRE colleagues, hospital teams, Community Social Work/NHS services, and Brokerage to coordinate support when ongoing needs are identified.
You will also be required to keep various data spread sheets updated with all customers information.
