Overview

Tissue Viability Nurse Specialist Hospital/Community | Band 7 | North London | 37.5 hours/week / London Supplement paid

Our Tissue Viability Team provides specialist care to patients with complex wounds, and plays a key role in promoting and educating colleagues in tissue viability and wound care. We are looking for someone with specialist tissue viability knowledge and experience in wound care service delivery to utilise and expand their skills in a senior role within our team. You need to be motivated, flexible, work independently and in a team, and enjoy working in a variety of environments - in people’s homes, hospitals, care homes, GP practices.

You will receive professional learning and development support and will become part of a team committed to the professional development of self and others. This is a 37.5 hours/week post with usual hours Monday–Friday (excluding weekends and bank holidays). Job share/part time would be considered. Main duties of the job

* To take a leading role in the management, development and efficient running of the Tissue Viability Service (TVS) across Whittington Health which includes Whittington Hospital, Haringey and Islington community and Haringey and Islington Mental Health in-patient units in North London Foundation Trust.
* To act as a resource of expert advice, carry own caseload and provide coordination of care from diagnosis, treatment and follow up care. To provide clinical leadership and develop patient-centred evidence-based care to positively affect patient outcomes.
* To provide specialist tissue viability advice, support, education and training to patients, carers and health care professionals based on evidence, current research, good practice guidance and policy development from DH, NMC and other national/professional bodies.
* To develop, promote, advance and monitor standards of care following national guidelines and ensure all national data requirements are collected.
* To work collaboratively with the Leg Ulcer Services and Lymphoedema services, and with all members of the Multi-Disciplinary Team.
* The post holder will work across Whittington Health to ensure full integration of the tissue viability service across the Integrated Care Organisation.

Job responsibilities

MAIN DUTIES

Leadership

* Provide leadership to the TV team to ensure the service is delivered effectively and within budget.
* Ensure all advice is evidence based and aligned with Whittington Health and national policies and guidelines.
* Provide clinical and professional leadership to support and inspire colleagues in local nursing homes, hospital wards, community teams and other care settings regarding management of patients with complex wounds.
* Promote clear, concise, and specific documentation related to tissue viability issues for audit and outcomes.
* Promote inter-professional working across the service via face-to-face contact, I-pads, teleconferences and virtual wards.
* Mentor and support junior team members and students to develop clinical and theoretical knowledge.
* Participate in the tissue viability education training plan to educate and develop staff.
* Triage referrals and allocate to team members; ensure tissue viability cover in the absence of the Lead Nurse.
* Act as role model in the absence of the Lead Nurse and support the Lead Nurse with the hospital pressure-relieving equipment and bed contract.

Managing Care

* Overall responsibility for delivering care to patients with complex wounds, reflecting the NMC code.
* Provide expert practice including assessment, initiation of treatment, interpretation of investigations and prevention strategies.
* Assess patients holistically and plan, deliver and evaluate care using evidence-based advice.
* Coordinate referrals across primary and secondary care to implement holistic treatment plans.
* Empower patients through clear communication and support independence where appropriate.
* Maintain accurate care plans in patient records and ensure adherence to pressure ulcer prevention policies and infection control best practice.
* Promote health and safety and report incidents per policy.

Managing Resources

* Teach and supervise clinical practice and identify training needs of staff.
* Line manage and support staff per HR procedures, including regular 1:1s and annual appraisals.
* Manage service budget and resources, including wound dressings and temporary staff, with cost-effectiveness in mind.
* Represent the TVS at meetings and ensure organisational arrangements support service aims.
* Maintain IT capabilities to produce reports and training programs.
* Oversee the pressure-relieving equipment contract on the hospital site to support the Lead Nurse.

Service Development

* Develop and implement policies, protocols, guidelines and care pathways for safe, equitable practice.
* Lead on national recommendations and guidelines related to leg ulcer and complex wound care management and prevention.
* Initiate research and audits to promote and develop patient care and disseminate findings to clinicians and GP commissioners.
* Organise and evaluate practice via audits to determine service efficacy.

Quality

* Prioritise referrals and ensure service provision meets individual needs while keeping the 6Cs at the centre of care.
* Participate in policy, standard setting, quality initiatives and clinical audit; uphold clinical governance standards.
* Ensure care meets Whittington Health standards and promptly report and escalate incidents as required.
* Attend clinical supervision; supervise others in line with policy.

Professional development and responsibilities

* Act as advisor/resource person to staff with specialist expertise.
* Participate in training for students and trainees; engage in resource projects and health promotion as requested.
* Undertake duties delegated by the Service Lead and monitor mandatory training within time scales.


Person Specification

Education / Qualifications

* Registered Nurse
* Post-registration qualification in Tissue Viability / Leg Ulcer
* Clinical degree or equivalent

Knowledge, Skills & Abilities

* Developed knowledge of tissue viability and complex wound care
* Ability to assess, direct and coordinate programmes of care
* Evidence of assessment and management skills for complex wound care

Experience

* Minimum of 3 years post-registration experience with at least 2 years at Band 6 level
* Sound clinical knowledge and judgement; up-to-date knowledge of recent research and national initiatives in tissue viability

Personal Qualities

* High level communication skills and ability to manage workload
* Flexible, efficient, good team worker with interpersonal skills
* Understanding of equal opportunities/equal access; experience in mentoring
