Are you passionate about supporting adults to live independently and improve their wellbeing? We’re looking for a dedicated Social Care Worker to join the Adult Front Door team within Worcestershire County Council’s People Directorate.
This is your chance to make a real difference at the first point of contact – where early intervention, strength-based conversations, and positive outcomes begin.
? What You’ll Be Doing:
1. Working with adults and carers using a strength-based approach, empowering them to live independently and access the right support.
2. Providing an effective telephone consultation and referral service for adults seeking support or advice about adult social care.
3. Supporting people in times of crisis, helping them regain control and plan for the future.
4. Engaging with internal and external professionals to create a joined-up, multi-agency approach.
5. Identifying safeguarding concerns and contributing to robust adult protection processes.
6. Supporting the delivery of Care Act 2014 responsibilities and understanding the implications of the Mental Capacity Act 2005.
? What We’re Looking For: You’ll have:
7. Significant experience in adult social care or health, particularly supporting people with learning disabilities, mental health needs, dementia, and long-term conditions.
8. Proven experience in assessing needs, developing support plans, and contributing to person-centred outcomes.
9. Confidence in engaging with individuals, carers, and professionals in a thoughtful, sensitive, and proactive manner.
10. Strong IT and record-keeping skills, with the ability to maintain accurate and detailed case notes.
11. A Level 3 qualification in Health and Social Care (or equivalent experience).
12. A commitment to equality, anti-oppressive practice, and ongoing professional development.
? You’ll Also Need:
13. Excellent knowledge of safeguarding, the Mental Capacity Act, and how to access community and universal services.
14. Experience in quality assurance, auditing, and evidence-based practice.
15. Strong organisational, communication, and problem-solving skills.
16. The ability to travel throughout Worcestershire, sometimes to areas not served by public transport.
