Reablement is a short-term, intensive care service designed to help people regain skills and confidence after a hospital stay, illness, or decline in health. The Role - Reablement Care Worker (Portsmouth Area) You'll be visiting service users in their homes, helping them safely regain independence by: Encouraging mobility, personal care, and safe use of the home environment Supporting meal prep, medication prompts, and daily routines Promoting confidence through goal-setting and progress tracking Empowering service users to reduce reliance on long-term care Areas Covered: You'll be supporting service users across the following postcode areas: PO1, PO2, PO3, PO4, PO5, PO6, PO7, PO8, PO9, PO10, PO11, PO12, PO13, PO14, PO16, SO31, SO32, SO33, GU31, GU32 Own transport is essential due to the mobile nature of the role. Requirements: In-date Care Certificate Experience in domiciliary care, hospital discharge teams, or community-based care Full UK driving licence own car Ability to work flexibly and independently Passion for helping people regain control of their lives Why Join SENDhelp?
