Job Summary
We are looking for a talented individual who is keen to learn and be a proactive member of the Urology team within Hywel Dda. You must be confident and able to demonstrate excellent verbal, written and IT communication skills, as well as having good organisational and planning skills. In this rewarding role, you will work closely with our clinical nurse specialists (CNS), consultants, and wider multidisciplinary team (MDT), to support the delivery of safe, effective, and patient‑centred care across a variety of outpatient and diagnostic settings. The Assistant Practitioner role is pivotal in supporting the safe and holistic care of our patients and involves providing telephone and face‑to‑face advice to patients, families and carers. Under the supervision of the Urology CNS Team, this person will also be responsible for providing follow‑up support for patients and families who are living after treatment or with a cancer diagnosis. This will involve signposting to other hospital and community services.
Main duties of the job
Information, Education & Support
* Provide clear, compassionate explanations of tests, procedures, and clinic processes.
* Offer reassurance, practical advice, and signposting to specialist services or resources.
* Support patients living with urology conditions, including those undergoing cancer and long‑term condition pathways.
Clinical Documentation & Administrative Duties
* Accurately update electronic patient records, documenting assessments, observations, and outcomes.
* Support management of clinic flow, appointments, and coordination with multidisciplinary colleagues.
* Assist in monitoring stock levels, equipment maintenance, and ordering supplies.
Patient Assessment & Clinic Support
* Undertake delegated clinical assessments, observations, and holistic reviews under the supervision of a registered practitioner.
* Prepare patients for a range of urology outpatient procedures (e.g., flow rate tests, bladder scans, flexible cystoscopy pre‑assessment, prostate assessment clinics).
* Perform and record measurements such as uroflowmetry, post‑void residual bladder scanning, urinalysis, and other point‑of‑care tests.
* Assist registered clinicians during investigations and minor procedures, ensuring patient comfort, dignity and safety.
* Prepare clinical areas, equipment, and instruments in line with infection prevention & control standards.

Person Specification
Essential Qualifications and Knowledge
* Level 4 vocational qualification in a relevant health care associated subject / Certificate in Higher Education Health Care Support Worker
* GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
* Evidence of continuing personal development relevant to the role in health care
* Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines and all Wales Band 4 Assistant Practitioner Governance Framework
* Clinical knowledge which is relevant to the role
* Demonstrates understanding of the importance of following procedures and treatment plans
* Demonstrates an understanding of the legislation related to Safeguarding (e.g. adults/children, the Mental Capacity Act and Deprivation of Liberties) to keep adults within their care safe
Essential Experience
* Significant and recent experience of providing relevant clinical care
* Interpersonal and communication skills and the ability to manage/diffuse challenging or threatening situations
* Experienced in undertaking a range of clinical skills relevant to the post as per role scope of practice and the national governance framework (catheter care, monitoring and recording vital observations, reviewing risk assessments, monitoring and interpreting blood glucose, supporting an RN with wound care)
Desirable Qualifications and Experience
* Local knowledge of statutory and voluntary services
* Teaching or assessment qualification
* Experience in a range of clinical skills including palliative care
* For community roles: experience of providing care in a community or primary care setting
* Administration of medicines via oral, topical, SC, IM routes
* Skills in non‑complex wound care
Language Skills
* Welsh speaker (Level 1) – desirable
