An exciting opportunity has arisen for a motivated, enthusiastic, and knowledgeable Mental Health Practitioner to join the South Manchester Mental Health Liaison Team, based within Wythenshawe Hospital and Trafford General Hospital.
The Liaison Service has been established since 2010 and operates 24/7, in line with CORE-24 requirements.
You will work as part of an integrated multidisciplinary 24-hour mental health liaison team, providing clinical interventions focusing on relapse prevention, early discharge, and improving quality of life for referred patients.
Your responsibilities include:
1. Assessing individuals attending the A&E department, including Under 16s in Pediatrics Emergency Department, with highly developed risk assessments and management plans.
2. Providing services to wards and the Intermediate Care Setting, assessing individuals with complex needs, including organic and functional illnesses.
3. Supporting and training hospital staff on recognizing mental illness and appropriate actions.
4. Offering assessment, support, and care for individuals brought under section 136 of the Mental Health Act.
