Band 3 Community Support Worker – Community Response Team
We are looking for an enthusiastic Band 3 Community Support Worker to join our Community Response Team. The role supports patients across South Warwickshire, working from our bases in Stratford‑upon‑Avon and Leamington Spa.
Responsibilities
* Provide high quality, compassionate personal care and associated duties under the direction of registered staff, without direct supervision.
* Assist patients to remain independent in the community and support safe discharges from hospital.
* Work closely with social services, GPs, community nursing teams, patients, relatives and carers.
* Contribute to the multidisciplinary team ensuring the delivery of safe, effective and compassionate patient care.
Qualifications & Requirements
* Valid driving licence and access to a personal car.
* Band 3 level experience as a clinical or community support worker.
* Strong dedication to providing compassionate care and maintaining high standards.
Hours & Conditions
The Community Response Team operates 08:30–21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated on a pro‑rata basis and include a mix of early, late and long days.
