Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for an Adult Advanced Social Work Practitioner / Mental Health Discharge Coordinator to work full time for Worcestershire.
The salary for this ASWP Mental Health Discharge Coordinator job is up to £44,711 per annum.
Responsibilities:
1. Review practices and processes, develop practice guidance and tools to support frontline staff.
2. Work as a senior and autonomous social worker responsible for own professional actions and the staff supported within their area of specialism.
3. Provide support to social workers in the Community Teams, critically analysing, supporting, and challenging social workers to improve quality and professional development.
4. Provide social work leadership, promoting the social perspective within inpatient settings, focusing on recovery and advocacy for individuals.
5. Offer advice and guidance on complex social care and housing matters related to individuals on the wards.
6. Collaborate with inpatient managers across the NHS trust, identify new admissions, and provide updates on discharge progress.
7. Oversee work with adults experiencing or at risk of abuse or neglect, undertake complex safeguarding enquiries, and manage threats or aggression.
8. Address oppression and discrimination, applying law to protect and promote people's rights, recognizing legal constraints and opportunities.
9. Lead mental health promotion, awareness of social stigma and disability, and act as an information resource for individuals, carers, and professionals.
10. Promote equality, dignity, and respect in all duties, in line with the Council's Equality & Diversity Policy.
Requirements:
1. Social Work Qualification or equivalent.
2. Understanding of relevant legislation.
3. Registered or eligible for registration.
