Community Support Worker - DIST South East

Hiring closing date: 27 April 2026.

Assisting qualified nursing staff in providing care and support of people with a diagnosis or suspected diagnosis of dementia and their carers, within the acute trust, mental health trust, community settings, and/or patient's own home.

Providing person‑centred care and working within the recovery model approach enabling patients to work towards their goals within their care plans.

The role is to support admission avoidance, including working directly with Primary Care services, Social Care services, Third Sector Organisations, non‑statutory care providers and local GP's. It also includes working directly within the Acute Trust and Inpatient Mental Health Services to support timely discharge and post‑discharge support.

The post holder is responsible for reporting regularly to the Team Lead or other qualified staff, working both under supervision and on their own, and reporting any relevant changes in the patient’s condition and environment.


Main duties of the job

Support admission avoidance and collaborate with primary, social, third‑sector and GP services; support timely discharge and post‑discharge care within the acute and inpatient mental health settings.

Act in a manner that protects and maintains the patient’s sense of dignity, personal relationships and social contact, respecting religious beliefs and cultural practices.

Assist qualified nursing staff in caring for people with dementia and their carers in acute trust, mental health trust, community settings and/or patients’ own homes.

Provide leadership and mentoring to student nurses.

Understand the Mental Capacity Act.

Access and maintain all mandatory training via OLM.

Attend appropriate courses as agreed at appraisal and be appraised once a year (lifelong learning).

Undertake clinical supervision to enhance practice and accept constructive criticism to benefit safe practice.

Job responsibilities

Please note: you must either drive or have access to a car to fulfil the duties of the role.

The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community and voluntary sector organisations to improve health and wellbeing and reduce inequalities.

As part of the HCA Academy, the successful candidate will attend a 5‑day face‑to‑face induction programme at the MSEICS Education Hub at Brentwood Community Hospital and a 5‑day EPUT corporate/mandatory training welcome (face‑to‑face or virtual).

Person Specification – Knowledge

* Basic understanding of dementia conditions and their impact on daily life.
* Experience recording service‑user activity on a clinical information system.
* Basic IT skills (Word and Outlook).
* NVQ qualification – level 3 or equivalent.


Person Specification – Skills and Experience

* Experience dealing with confidential and sensitive information.
* Understanding of working with people with dementia and appreciation of vulnerability.
* Awareness of patients’ mental health issues, physical health care needs and wellbeing initiatives.


Person Specification – Additional Qualities

* Current driving licence and access to a vehicle for work purposes.
* Ability to travel across sites and Trust boundaries to attend meetings.
* NVQ3 in Health or Social Care or equivalent experience.
