Job overview Are you an innovative Physiotherapist seeking a specialist community role with system impact? Join the Provider Improvement and Response Team (PIRT), improving outcomes for residents in care and nursing homes across Staffordshire and Stoke-on-Trent. This permanent post is initially aligned to a 12-month project supporting care home quality improvement and intensive support. The role may evolve in line with service needs. Secondment applications are welcome, subject to manager agreement. About the role You will work autonomously within a multi-disciplinary team, supporting care home providers to improve safety, wellbeing and outcomes. You will undertake holistic assessments, deliver targeted interventions, build staff capability and contribute to admission avoidance. You will also support system learning, quality improvement, and work with partners across Local Authority, primary care, acute and voluntary services. About you Clinically confident with experience supporting older adults/complex needs Community experience, ideally with care homes Strong communication and problem-solving skills Committed to quality improvement and collaborative working Flexible to travel You will receive clinical supervision via MPFT CHIST and AHP services. Advert Build and maintain effective relationships with care homes, partners, and multidisciplinary teams to support high-quality, coordinated care. Work across organisational boundaries to identify risks, address challenges, and support improvements in care delivery and resident outcomes. Undertake holistic assessments of residents’ needs where required, working with care home staff to strengthen care planning and support. Provide advice, guidance, and practical support to care home staff across areas such as safety, falls prevention, infection control, and wellbeing. Contribute to reducing avoidable hospital admissions through proactive identification of deterioration and early intervention. Support education, training, and upskilling of care home staff to improve confidence, capability, and quality of care. Identify themes, risks, and barriers within services, escalating concerns and contributing to system-wide learning and improvement. Maintain accurate, timely records and ensure effective communication across teams and organisations. Contribute to service development, projects, and quality improvement initiatives within the local health and care system. Work autonomously with a high level of professionalism, demonstrating adaptability, sound judgement, and effective workload management.
