We are pleased to present a rare opportunity for a qualified Advanced Clinical Practitioner (ACP) to work within the Acute Medical Same Day Emergency Care (SDEC) and Assessment Unit. We seek an experienced ACP eager to develop this service while working clinically. The role involves autonomous practice within a wider medical team, including assessing, requesting and interpreting investigations, diagnosing, and creating management plans. It requires complex diagnostic decision-making, prescribing, treatment planning, and managing high-risk situations safely and decisively. The Lead ACP will play a key role in developing and enhancing our SDEC pathways and the care provided to acutely unwell patients. Adaptability, high standards, and initiative are essential qualities for this role.
Key Responsibilities
1. Clinical Leadership: Provide expert clinical decision-making, manage complex cases, and lead advanced assessments, diagnostics, and treatment plans for undifferentiated patients. Lead the ACP team within ACM.
2. Service Development: Collaborate on designing and implementing innovative care pathways aligned with national guidelines and UHD priorities.
3. Education & Mentorship: Foster a learning culture by mentoring junior ACPs, nursing staff, and students.
4. Quality Improvement: Lead audits, research, and governance initiatives to improve patient safety, reduce hospital admissions, and enhance care continuity.
5. Multidisciplinary Collaboration: Work closely with consultants, GPs, social services, and community teams to optimise care coordination and discharge planning.
6. Strategic Influence: Contribute to Trust-wide strategies, policies, and service redesigns for older persons’ care, advocating for patient-centred approaches.

UHD is investing in services across the Trust, many of which are being transformed and developed following the merger and the New Hospital Programme. Please note that if the role or service relocates as part of a planned move, employees will not be eligible for excess mileage reimbursement. Travel from home to the new work base will be considered a normal commute. Other changes unrelated to relocation will be managed according to relevant Trust or national terms and conditions.
