We are recruiting for a full-time Band 7 Senior Hospital at Home Allied Health Professionals to join our Brent Unplanned Care Team, based at Sudbury. This is an exciting opportunity to play a key leadership role within an innovative service that delivers high-quality, hospital-level care directly in patients' homes.

As a Senior Hospital at Home Practitioner, you will work with a high level of autonomy while collaborating closely with Acute Trusts, the Adult Community Nursing Team, and a range of health and care partners, including statutory and voluntary agencies, to deliver excellent, person-centred care.

You will work on a rotational shift basis within the Brent Hospital at Home service (8am‑8pm, seven days a week) and act as Team Leader on designated shifts, taking responsibility for day‑to‑day service delivery. You will also hold delegated responsibility for specific areas of clinical practice or operational management, contributing to service development and quality improvement. The role requires strong prioritisation skills and the ability to manage a varied workload with minimal supervision.

This role is ideal for an experienced practitioner who is passionate about leadership, innovation, and delivering high-quality acute care in the community.


Main duties of the job

Undertake comprehensive health assessments, including physical examination and the ordering and interpretation of appropriate diagnostic tests for patients within the Hospital at Home service.

Use advanced clinical assessment and decision‑making skills to coordinate care planning and initiate timely referrals for diagnostics and specialist input where appropriate.

Contribute to the establishment, delivery, and leadership of a comprehensive Hospital at Home service within community services.

Practise autonomously as an advanced practitioner, assessing, diagnosing, and treating patients safely and effectively in a timely manner.

Demonstrate sound clinical judgement and critical decision‑making in complex and evolving clinical situations.

Actively support the delivery of key performance indicators and contribute to strategic objectives, service development, and continuous improvement initiatives.

Take responsibility for the day‑to‑day management of a defined caseload, ensuring high standards of quality, safety, risk management, and performance.

As a non‑medical prescriber, prescribe in line with national and local policies, within scope of practice and under Patient Group Directions where applicable.

Provide clear, compassionate information to patients and families to support understanding of disease processes and enable self‑management.

Person Specification


Education/Qualification

* Health care professional with current registration (Physiotherapist, Occupational Therapist, Paramedic )
* Evidence of continuing professional education and training at the master's Level. ? Further desirable qualifications in Primary Care/ Accident & Emergency/ acute clinical assessment
* Further desirable qualifications in Primary Care/ Accident & Emergency/ acute clinical assessment.
* Qualification in teaching or practice‑based learning.


Experience

* Extensive experience in Primary/Secondary Care and/ or Accident & Emergency.
* Proven experience in utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and manage patients presenting with undifferentiated and undiagnosed conditions
* Experience in assessing and providing quality care to people with Long Term Conditions
* Track record of practice excellence.
* Experience in effectively working in collaboration, negotiating, and liaising across organisational boundaries
* Experience in monitoring clinical standards
* Proven ability to prioritise and manage own workload with minimal supervision
* Experience determining the best ongoing care for the patient
* Experience in team/service/ staff management.
* Experience of implementing evidence-based care and developing others to do so
* Experience of conducting research projects and audit.
* Experience of change management and implementing change in practice
* Evidence of innovation in practice.


Skills & Knowledge

* Evidence of a comprehensive range of clinical skills and expertise relevant to the role
* Competent in physical examination of ENT, eye, gastrointestinal, respiratory, cardiovascular, neurological, skin, musculoskeletal systems, and wound management
* Confident in assessing acute clinical presentations
* Able to deal with conflict and manage appropriately
* Basic IT skills, including use of Microsoft Office package, EMIS, and email
* Specialist knowledge in clinical reasoning, history taking, physical examination, diagnostic skills and prescribing/treatment.
* In depth knowledge of pharmacology
* Advanced knowledge and in depth understanding of anatomy and physiology of: ENT, eye, gastrointestinal, respiratory, cardiovascular, neurological, skin, musculoskeletal systems and wound management.
* Knowledge of acute clinical presentations and management of acute conditions.
* Clinical knowledge across a range of long term and enduring conditions
* Knowledge of current NHS organisational structures and policy initiatives.
* Knowledge of recent developments in NHS
* Knowledge of monitoring service outcome measures.
* Knowledge of adult protection and safeguarding children's issues.
* Knowledge of HCPC/NMC Code of Professional Conduct.
* Knowledge of Personal Development Planning
* Knowledge of current health care policy and legislation and relevance to the delivery of healthcare services
* Detailed knowledge of Primary Health Care and collaborative working with other care providers, e. g. social care and voluntary services
* Knowledge of the audit process and ability to implement in practice
* Knowledge of evidence‑based practice and able to implement in practice
* Knowledge of clinical governance processes, assessing and managing risk
* Knowledge of clinical supervision
* Knowledge of how to implement equal opportunities in practice
* Able to lead and motivate a team and implement change
* Knowledge of resource management.


Key Attributes

* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
