We are looking for an enthusiastic new colleague to join our friendly and experienced team, supporting us with our Patient Administration and Patient Care Navigation. This is a permanent role, based on 18hr - 25hr per week over x4 afternoons - Mon, Tues, Wed & Friday.
We very much welcome applicants with previous experience of working in a GP practice. You need to be organised, have strong IT skills, and be able to converse with patients confidentially for this role

Job Summary
The following are the core responsibilities of the Practice Administrator & Care Navigator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
1. Process all clinical correspondence—including paper documents, electronic records, and emails—and ensure they are accurately attached to the relevant patient records.
2. Accurately code diagnoses, procedures, investigations, and other specified clinical actions using SNOMED codes, in line with practice protocols.
3. Forward relevant matters to GPs, clinical team members, or non-clinical staff as appropriate.
4. Recognise complex or high-risk issues—such as safeguarding concerns—that require escalation to the clinical team.
5. Always adhering to established practice protocols.
6. Support patient navigation during peak times of the day. This will include:
a. Utilising the practice appointment and telephone systems to efficiently direct and book patients in for care with the appropriate clinician. Answer incoming phone calls, transferring calls or dealing with the callers' request appropriately.
b. Signposting patients to the correct service.
c. Initiating contact with and responding to, requests from patients, team members and external agencies.
d. Manage all queries as necessary in an efficient manner.
It would be ideal if you currently work in a GP practice or have recent experience working in a primary care service. We can offer full training should that not be the case and you feel that you have excellent IT skills and will learn the ropes quickly.
You would be an integral part of our friendly and efficient team, helping to make sure the practice runs smoothly.
Training on our IT systems and processes would be given.
This role is subject to a clear DBS check, which we would fund. We also require two work related references.
