Overview

Job Summary

Work as part of the multidisciplinary team delivering care to patients with support and supervision provided by a registered health care professional. Undertakes personal care activities and clinical care duties with minimal support and supervision as delegated by a registered healthcare professional in accordance with the HEIW All Wales Guidelines for Delegation. Ensures the registered healthcare professional is regularly updated on the patients/client's condition and progress.

Contributes to the teaching and supervision of students and junior staff. Delivers role in accordance with Health Board/Trust Values and Code of Conduct for HCSW's in Wales.


Responsibilities

* Assist patients in line with agreed care plans, reporting observations and changes promptly to enable monitoring and adjustments.
* Obtain informed consent before interventions and act appropriately if care is declined, advocating for patient needs where necessary.
* Maintain strict infection prevention standards through effective hand hygiene, environmental cleanliness, and equipment care.
* Support patients with washing, bathing, dressing, oral hygiene, toileting, and safe waste disposal, ensuring privacy and dignity. Use mobility aids such as hoists and wheelchairs when required.
* Assist patients with eating and drinking, including food choice, preparation, and feeding support where appropriate.
* Provide pressure relief as directed, monitor skin integrity, and report abnormalities promptly.
* Promote independence and therapeutic relationships through meaningful activities tailored to patient needs.
* Accurately record care activities, physiological measurements, and relevant patient information in appropriate systems.
* Undertake and record vital signs, escalating concerns using Early Warning Score protocols.
* Perform basic tests (e.g., urinalysis, blood glucose monitoring), ensure quality control, and report abnormal results.

Job Description – Job responsibilities

* Planning and Design
Plan and prioritise own delegated workload, ensuring tasks are completed efficiently, safely and effectively, prioritising based on patient needs and emergencies.
* Work within established procedures under the supervision of a registered healthcare professional.
* Obtain diagnostic samples (e.g., urine, faecal, sputum, wound swabs, venous blood) ensuring proper handling and infection control.
* Undertake delegated care duties as directed by a registered healthcare professional (e.g., ECG, catheter care, stoma care, wound dressings, cannula removal).
* Escort patients to other departments within the healthcare setting under delegation; act as a chaperone when appropriate.
* Ensure safety of patients requiring increased supervision and report changes promptly.
* Adhere to safeguarding principles and report concerns promptly.
* Participate in clinical emergencies as part of the MDT within scope of practice.
* Act on own initiative within scope of practice and report observed changes in patient needs.


Stock and Equipment

* Maintain stock, equipment and materials and report when below standard.
* Maintain cleanliness of equipment and environment; manage disposal of linen and clinical waste.
* Check functionality of equipment and report issues.


Communications

* Provide routine information to patients, carers and staff; overcome barriers using empathy and tact.
* Communicate effectively with the MDT; relay instructions and feedback as required.
* Respond to routine queries and manage messages and information flow.
* Promote health education in line with policies.


Improvement and Monitoring

* Follow legislation and policies; contribute to service efficiency and patient experience.
* Adhere to SOPs and policies; lone worker policies in community settings as applicable.
* Contribute to audits, quality improvement, and clinical trials as required.


Management, Leadership and Training

* Supervise Band 2 Nursing Healthcare Support workers and apprentices.
* Work with student nurses and placements; delegate tasks to ensure service delivery.


Digital and Information

* Maintain accurate written and electronic records; comply with data governance and confidentiality.
* Demonstrate proficiency with digital tools for tasks and training as appropriate.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome.


Person Specification

Essential

* Qualifications & Experience: Level 3 qualification in a health care related subject or equivalent experience with evidence of work-based competency.

Desirable

* Experience of working in a relevant health or social care environment.
* Welsh language skills (desirable levels 1-5).
