Role overview
The EIV pathway delivers crisis management to patients living in Herts Valleys and aims to prevent hospital admission. It provides a clinical assessment by a nurse or paramedic jointly with a Healthcare Assistant. The operational hours of this pathway are currently 08:00 – 20:00, 7 days a week (subject to change).
The EIV pathway is aligned to the Unplanned Care service, which delivers clinical care to patients in their own home at times of crisis. The Rapid Response (RR) pathway aims to prevent hospital attendance/admission, and the Discharge to Assess (DTA) pathway facilitates early discharge from hospital. The Unplanned Care team operates from 08:00 to 22:00, 7 days a week. The EIV pathway will supplement and support the delivery of RR and DTA in Unplanned Care.
Responsibilities
* Provide skilled, effective and evidence-based clinical care to patients in a variety of community settings.
* Enable emergency provision of equipment through effective prescribing and ordering.
* Make risk assessments, establish the safety or otherwise of patients’ ability to stay at home, and arrange necessary nursing and supportive packages.
* Identify safeguarding concerns and effectively manage concerns or refer for safeguarding investigation. Apply knowledge of safeguarding processes and policy.
* Provide autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/or complex health needs and long-term conditions.
* Assess and manage risk, intervene proportionately, and protect people from harm while upholding human rights. Apply knowledge of the Mental Capacity Act and Care Act.
* Provide evidence-based clinical/therapeutic interventions based on best practice to improve health outcomes and promote choice.
* Work in close collaboration with other community agencies, including Locality planned care teams, In-Patient services and GPs.
* Deliver agreed clinical quality and performance standards within the Unplanned Care team.
Qualifications and expectations
* Demonstrated ability to provide clinical care in community settings and manage complex health needs.
* Ability to assess risk, apply safeguarding policies, and work within the Mental Capacity Act and Care Act frameworks.
* Evidence of collaborative working with multi-disciplinary teams and primary care.
* Evidence of appropriate professional registration and ongoing commitment to professional development.
