We're bringing together nurses, therapists, health and therapy assistants to work alongside acute multi-disciplinary teams and transfer of care hubs to promote an efficient, effective hospital-to-home transition for our patients.
Our multi-disciplinary Home First teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients discharged from hospital into Home First receive step-down intermediate care, involving community-based assessments and interventions provided in their own homes to support recovery and prevent re-admission. Home First also provides acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

As a Health and Therapy Assistant, you will provide personal, health, and therapy care for people across Sussex with diverse needs. Your role will be varied and engaging, supporting individuals to achieve greater independence. You will assist multidisciplinary colleagues in ongoing assessments and care provision, including simple clinical procedures such as taking observations, venepuncture, administering medications, wound care, rehabilitation, and equipment provision.
No previous experience is necessary; we will train you on the job and support you to achieve the Care Certificate. Our comprehensive competency framework will help you develop continuously. You need to be able to drive with a full UK license and have access to a vehicle due to the geographic area covered by the team. With experience and further training, you can progress to roles such as Nursing Associate or other degree-level healthcare professions like Nurse, Podiatrist, Midwife, or Therapist. There are many opportunities for career advancement with us!
