We are expanding our City Community Mental Health Team and are seeking motivated and compassionate Band 6 Community Care Coordinators to join our Community Mental Health Team in Bradford.
This role offers an exciting opportunity to make a real difference in the lives of service users by delivering high-quality, person-centred mental health care.
Main duties of the job
Key Responsibilities:
* Coordinate and deliver community mental health care in line with Trust policies.
* Undertake comprehensive assessments and manage risk effectively.
* Work collaboratively with service users, carers, and partner agencies.
* Administer and monitor medication in accordance with NMC and Trust guidelines.
* Participate in case conferences and contribute to service development.
* Promote equality, diversity, and safeguarding in all aspects of care.
Qualifications
* Professional qualification in mental health nursing, social work, occupational therapy.
* Degree level education or equivalent experience.
* Registration with a professional body (e.g., NMC, SWE, BAOT).
Experience
* Experience as keyworker with a mental health client group.
* Relevant experience in mental health care.
* Experience of being a clinical adviser to junior staff or students.
Person Specification / Requirements
Right to work in the UK – applicants must hold the legal right to work in the United Kingdom throughout the duration of employment. For those requiring a visa to work in the UK, sponsorship is available only for registered healthcare practitioner roles. Continuous employment requires the holder to maintain and promptly notify the Trust of any changes to their right‑to‑work status. Failure to comply may result in termination.
