About the role
* Provide holistic, patient-centred care, including symptom management, advanced care planning, and supportive assessments.
* Work within a multidisciplinary team to reduce avoidable hospital admissions and facilitate patient choice regarding place of care.
* Support patients, families, and junior colleagues with guidance, mentorship, and education.
* Participate in service development, audits, and reflective practice to ensure high-quality, evidence-based care.
* Represent St Luke’s positively in all interactions, upholding our values and professional standards.
Who we’re looking for
* Is registered with the NMC and has post-registration experience (several years preferred)
* Has experience in palliative, end-of-life, community, or specialist care (e.g., Oncology, Long-Term Conditions)
* Can work effectively in multi-professional teams across healthcare boundaries
* Demonstrates strong communication, prioritisation, decision-making, and IT skills (Word, Excel, Outlook)
* Is committed to safe, high-quality, evidence-based practice
* Is calm, approachable, and professional, with a friendly and supportive team ethos
* Acts with integrity, honesty, and accountability
