What you will do:
Facilitate timely and safe discharges from hospital environments for adults, while managing unplanned situations at home.
Conduct a variety of assessments including functional, Care Act, safeguarding, Mental Capacity Act, and Continuing Health Care assessments.
Drive strength-based and person-centred practices, enabling people to thrive in their community by utilizing and building upon their own networks and strengths.
Collaborate with a multidisciplinary team including Social Workers, Community Assessment Officers, Reablement Coordinators, and Occupational Therapists.
Form successful partnerships with acute, primary, and community health care sectors, alongside voluntary and independent organisations, to promote health and well-being in line with the Care Act.
Deliver personalised care plans tailored to adults' care and support needs, ensuring adherence to national legislation and safeguarding principles.
