The Role: As a Complex Care Nurse Case Manager, you will be pivotal in designing and implementing person-centered care plans, coordinating services from multiple disciplines (GPs, therapists, social workers), overseeing care packages, and driving continuous improvement through data analysis. You'll also play a role in local business development and commissioning.
