We are busy town centred practice, that deals with a mixed list.
Also supported from the PCN, a pharmacist, pharmacy technician, physiotherapist, social prescriber, mental health nurse, community matron with CCT team.

JobSummary:

ClinicalResponsibilities
* In accordancewith the Practice timetable, as agreed, the post holder will makethemselves available to undertake a variety of duties including surgeryconsultations, telephone consultations and queries, visiting patients athome, issuing and signing repeat prescriptions and dealing with queries,paperwork and correspondence in a timely fashion.
* Makesprofessionally autonomous decisions in relation to presenting problems,whether self-referred or referred internally within the Practice.
* Receivespatients with undifferentiated and undiagnosed problems and makesassessment of their health care needs.
* Screens patientsfor disease risk factors and early signs of illness.
* Develops careplans for health in consultation with patients and in line with currentPractice disease management protocols.
* Providescounselling and health education.
* Prescribes inaccordance with theCCG prescribing formulary whenever this is clinicallyappropriate.
* Admits ordischarges patients to and from caseload and refers them to other careproviders internally or externally as appropriate.
* Supports thetrainers in teaching in the Practice and gives clinical supervision totraining doctors.
* Complies withrelevant Practice policy/guidelines, e.g. Child Protection, confidentiallyissues etc.
* Is committed tolifelong learning, audit and effectiveness issues to ensure evidence basedbest practice.
* Maintains GMCregistration.
* Ensurescontinuous Medical Defence cover.
* Ensuresinclusion on a local Performers list.
OtherResponsibilities within the Practice
* Consults withpatients in the surgery and at home visits as agreed internally betweenall partners.
* Be available foron-call duties to answer patients telephone queries or carry outemergency home visits, or surgery consultations as appropriate.
* Contributes tothe evaluation/audit and clinical standard setting with colleagues.
* Records clearand contemporaneous I.T. based consultation notes to agreed standards;
* Compiles andissues computer-generated acute and repeat prescriptions (avoidinghand-written prescriptions whenever possible);
* In general the postholder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.
* Contributes tothe strategic management and development of the Practice e.g. indeveloping and contributing to Practice policies and guidelines.
* Attendance attraining, Practice Meetings and events organised by the practice or otheragencies, where appropriate.
* Meets all tighttimescales/deadlines for audits and written returns to ensure that thePractice meets quality standards and receives the designated funding (e.g.Quarterly Enhanced Services returns, annual QOF audit etc.)
* Ensuring fullaccreditation which will include full and unrestricted registration withthe GMC, membership of an appropriate defence body and delivery of annualevidence for appraisal or accreditation purposes in keeping with Nationalrequirements at any time.
