Lead with Purpose: Become a Weekend Deputy Team Manager in Southwark’s Hospital Discharge Transfers of Care Unit!
Are you ready to take on a leadership role that makes a real difference during weekends? Southwark Council seeks an energetic Deputy Team Manager to oversee the Hospital Discharge Transfers of Care Unit during weekends, working a 36-hour week from Tuesday to Saturday. This role offers leadership opportunities while ensuring smooth patient discharges and care continuity.
About The Role
You will lead and motivate social workers and healthcare professionals to deliver high-quality care, especially during busy weekends:
* Operational Leadership: Manage daily operations from Friday to Tuesday, overseeing referral triage, case allocation, and assessments.
* Guidance and Support: Offer expert advice on complex discharges, safeguarding, and mental capacity assessments.
* Collaborative Networking: Lead hospital discharges at Kings and St Thomas Hospital and manage referrals for step-down flats in extra care schemes.
* Team Development: Mentor and supervise staff, fostering continuous learning and confidence in autonomous team operation.
About You
We seek a dynamic leader committed to social care excellence, with:
* Deep Knowledge: Understanding of the Care Act, Mental Capacity Act, and Continuing Healthcare, with practical application experience.
* Proven Experience: Leading MDT and safeguarding meetings, providing reflective supervision, supporting professional growth. Experience as a Practice Educator or in ASYE is desirable but not essential.
* Leadership Skills: Confident leadership, creative thinking under pressure, decisive decision-making for resident outcomes.

The Hospital Discharge Transfers of Care Unit ensures seamless care transitions from hospital to home or other settings, managing discharges, safeguarding, and care planning with compassion. As a weekend leader, you will ensure services remain effective and responsive during peak times.
