As an Outreach Support Worker, you will play a vital role in supporting individuals with mental health needs and/or learning disabilities to live independently within the community. You will provide practical and emotional support tailored to each client’s individual needs, guided by person‑centred support plans and risk assessments.
Responsibilities
 * Support clients in maintaining their accommodation, assisting with daily living tasks such as personal care, attending appointments and ensuring access to appropriate health and social care services.
 * Provide tenancy‑related support and work in close partnership with external professionals, families and community services to promote overall wellbeing and independence.
 * Prepare person‑centred support plans with clients which reflect their views and aspirations and respect their beliefs and opinions, reviewing and amending them as necessary.
 * Support clients in fulfilling their aspirations, supporting them to live as independently as possible, arranging activities and outings, and developing skills and knowledge.
 * Communicate in an open and transparent way with clients and other team members, acting with integrity and doing what you say you will do.
 * Provide personal care to clients in a way that respects their choices and dignity, including assistance with washing and toileting.
 * Keep appropriate, accurate records, including daily notes, with full involvement of the client.
 * Ensure that all support and care are provided in a way that protects clients from abuse and exploitation, maintains their dignity and supports positive risk‑taking.
 * Assist with a range of household duties in accordance with the needs of the client, including cooking, cleaning, laundry and other domestic duties.
 * Carry out a wide range of personal care and domestic duties as required and provide appropriate level of care and support in accordance with the care plan.
 * Understand the importance of maintaining accurate written and numerical records.
 * Work cooperatively as a team member, liaising and communicating effectively with carers, families, inspectors and other external stakeholders.
 * Work always in accordance with Health and Safety requirements such as Manual Handling, Food Hygiene, Fire Safety, infection control and Risk Assessment procedures.
 * Be willing to learn and undertake relevant training and qualification in care if required.
 * Possibly provide physical support such as climbing stairs, using hoists, pushing wheelchairs, moving people and night work.
Qualifications
 * Care Certificate (essential) or willingness to undertake Care Certificate.
 * Previous experience working in care (desired).
 * Current driver licence (desirable).

 * Previous experience working in care (employment or carer at home).
