Job Summary
You will be responsible for the provision of skilled nursing care to the population of York Central locality. You will deliver care within the boundaries of your role, focusing on supporting patients to be healthy, monitoring long term conditions, health prevention and screening activities. You will work collaboratively to meet the needs of patients, ensuring the highest standards of care, supporting the delivery of policy and procedures. You must always work within your own competence and professional Code of the Nursing and Midwifery Council (NMC).
Main Duties
Clinical Responsibilities:
* Assess, plan, develop, implement and evaluate programmes of care to improve health and well-being and meet the healthcare needs of individuals.
* Perform and interpret spirometry (ARTP Registered).
* Perform routine nursing tasks: clinical observations, height/weight checks.
* Provide a holistic, evidence-based approach to care.
* Administer and monitor medication in line with protocols, standards and legislation.
* Educate patients, families and carers to develop knowledge about health and well-being and support self-management where possible.
* Prioritise health problems and intervene in complex, urgent or emergency situations, including initiation of effective emergency care.
* Assist in the provision of nursing care within the context of primary care for patients with mental health problems.
* Offer advice about adult and child vaccinations.
* Delegate aspects of care safely and assess competence.
* Ensure daily checks are recorded accurately and promptly (e.g., spirometry, Defibrillator, emergency equipment, oxygen).
Other Responsibilities:
* Recognise and work within own competence and in accordance with the NMC Code of Professional Conduct.
* Provide support to other clinical team members as required.
* Monitor safety and effectiveness of own clinical practice and maintain accurate documentation.
* Follow agreed protocols, referring to GPs as appropriate and follow local and national Infection Control guidelines.
* Assist with maintenance of equipment and manage stock levels; ensure equipment is stored securely when not in use.
* Assist in setting up the Community Diagnostic Centre at the start of the session and store equipment correctly at the end of the shift.
Business, Governance and Safeguarding
* Act as a positive role model and promote Nimbuscare ethos and policies.
* Adhere to Health & Safety requirements and Nimbuscare policies; participate in governance, educational and staff meetings; support service improvements.
* Build and maintain professional relationships with other healthcare professionals; safeguard and promote the welfare of adults at risk and children.
* Understand duties under relevant safeguarding legislation and alert the Nimbuscare Safeguarding Lead to any concerns.
* Participate in mandatory training and performance reviews; pursue ongoing professional development and share learning with the team.
* Communicate effectively within the team and with customers and carers; adapt communication methods as needed.
Person Specification
Physical Skills - Essential: VDU use.
Qualifications - Essential: Current Professional Registration (NMC); evidence of continuing professional development; qualification in performance and interpretation of spirometry with ARTP registration. Desirable: long term condition management (Asthma/COPD).
Skills and Knowledge - Essential: BLS trained; IT skills (MS Office / SystmOne / EMIS); excellent communication and interpersonal skills; ability to work autonomously and as part of a team; excellent time management. Desirable: evidence of statutory and mandatory training.
Personal Attributes - Essential: ability to prioritise, work under pressure, be flexible, self-motivated, and use own initiative.
Experience - Essential: minimum 1 year experience in a Primary Care setting; experience of multidisciplinary working. Desirable: experience using PGD; experience in adult and child immunisations.
