Community and Hospital Support Worker – Fixed Term

We’re looking for someone to work in our Hospital Discharge team. You’ll be working closely with integrated discharge hub staff to support customers to be discharged from hospital and those who need support to stay safe and well in their own homes in the community.
We will reimburse mileage and parking expenses for any travel undertaken as part of your role.
Job Responsibilities
* Complete tasks to help customers get home safely from hospital, such as property inspections, collecting belongings, dropping off food parcels, or arranging quotes for deep cleans so discharge can go ahead.
* Do ad‑hoc or regular visits to people identified in the community as needing further support and complete tasks as requested, e.g., checking homes are warm and safe, checking medication, food, and other essentials are available.
* Visit wards, engage with customers, and source volunteering services to support customers’ wants and needs.
* Collaborate with other teams within the hospital to ensure a timely discharge.
* This is a physically active role, where you can expect to walk approximately 5‑6 miles per day while delivering small food parcels.
What We’re Looking For
* Excellent communication skills.
* An aptitude for working with vulnerable and/or older people.
* Understanding of what great customer service is.
* Ability to work under pressure and manage a high caseload.
* Excellent ICT and administrative skills.
* Identifying issues to help prevent delays in discharge.
* Hold a full UK driving licence and use your own vehicle, including business insurance.
* Enhanced DBS required.
