Job Summary: The Advanced Clinical Practitioner (ACP) will provide high-quality, evidence-based care to patients across the full age range, including both paediatrics and adults. Working autonomously within their scope of practice, the ACP will assess, diagnose, plan, implement, and evaluate care for patients with undifferentiated and undiagnosed conditions, including those presenting with minor injuries and minor illnesses. The ACP will be an integral part of the multidisciplinary team, ensuring safe, effective, and efficient clinical practice in line with national and local protocols. They will also contribute to clinical leadership, service development, and provide mentorship and supervision to other clinical staff. Key Responsibilities: * Deliver advanced clinical care to patients in a variety of settings including primary care, urgent care, A&E, or GP Out-of-Hours. * Undertake comprehensive clinical assessments, formulate differential diagnoses, and initiate treatment plans. * Prescribe independently within scope of practice. * Manage and treat patients presenting with minor illnesses and minor injuries. * Provide care across the lifespan, including both paediatric and adult patients. * Work in accordance with established clinical protocols and national/local guidelines. * Participate in clinical governance, audit, service evaluation and quality improvement activities. * Mentor and supervise other clinicians and support their development through formal and informal education. * Actively participate in multidisciplinary team meetings and contribute to service development. * Maintain accurate, timely, and confidential patient records. Essential Criteria: * Registered Clinician * Master’s Degree in Advanced Clinical Practice (or equivalent or higher) * Qualified independent non-medical prescriber * Minor Injuries and Minor Illness qualification, with formal study in both areas * Minimum 5 years post-registration experience, including: * At least 2 years in primary care, A&E unscheduled care, or related field * Minimum 2 years’ experience as a Practitioner in one of the following: * General Practice * Walk-In Centre (WIC) * GP Out-of-Hours (OOHs) * Accident & Emergency (A&E) * Experience working to clinical protocols or guidelines * Proven ability to provide care across the age range (Paediatrics and Adults) * Experience offering mentorship and supervision to other clinical staff Desirable Criteria: (Add any role-specific desirable experience or qualifications here) Working Conditions: * The role may include evening, weekend, or on-call work. * Will require working under pressure in fast-paced clinical environments. * May involve travel across sites depending on service configuration
