Long Term Conditions Nurse at HMP Full Sutton, a Category A men's Prison in Full Sutton, York.
This rural site offers parking, on‑site gym, caf, team building days, well‑being events, regular 1‑1s and good scope to develop.
Hours
37.5 hours per week, may include night shifts. Two long days and two short days: 8:00‑16:00 or 7:00‑19:15, 4 shifts a week including one on 4 weekends.
Salary
Up to £49,175, depending on experience.

Candidates are required to have a legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
Main duties of the job
* Ensure comprehensive handover of services when patients transfer across wings or custodial care settings.
* Identify immediate health needs and any external services or prescribing requirements for prisoners prior to remand or repatriation.
* Facilitate arrangements with agencies to deliver complex care packages, fostering collaboration with in‑house services such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education.
* Provide proactive intervention to address improvement recommendations from CQC and HMPP inspections.
* Deliver continuity of care across health and social care agencies, from courts to custody and community, improving communication.

Qualifications required
* Registered Nurse – General with relevant NMC Registration
* Advanced Nurse Prescriber/Practitioner
* Educated to degree level
* Knowledge of health and social care models across the criminal justice system and national/local initiatives relating to physical health
* Knowledge of Evidence Based Practice and Clinical Governance
