We are seeking a dynamic and compassionate Tissue Viability Clinical Nurse Specialist to join our team at Hillingdon Hospital. This is a senior clinical role at Band 7, leading the delivery of a high‑quality, evidence‑based Tissue Viability service across the Trust.
As a key specialist, you will provide expert assessment and management of complex and non‑healing wounds, support frontline teams with professional advice, and contribute to Trust‑wide initiatives to reduce pressure damage and improve patient outcomes. You will champion excellence in wound care, drive education and improvement initiatives, and act as a role model for clinical leadership, embodying our CARES values.
The role includes managing referrals, guiding policy and formulary development, supporting safeguarding processes, and collaborating with multidisciplinary and external partners to deliver integrated, patient‑focused care. You will also lead and support audits, quality improvement, data reporting, and the implementation of best‑practice systems to enhance patient safety.

Main duties of the job
Deliver a high‑quality, evidence‑based Tissue Viability service, ensuring robust systems for monitoring, prevention and management of pressure ulcers and wounds.
Develop and implement strategies, tools, and processes that support the reduction of harm, improve patient outcomes, and strengthen clinical practice.
Lead the development and maintenance of comprehensive monitoring systems to track pressure ulcer incidence and prevalence, ensuring data is used effectively for improvement.
Provide a responsive referral service and specialist assessments for patients with complex or non‑healing wounds, supporting frontline staff to deliver safe and effective care.
Plan, deliver, and evaluate Trust‑wide training programmes on all aspects of Tissue Viability, integrating theory into practice through workplace‑based learning to maintain high standards across all wards.
Remain updated on national and local developments in Tissue Viability, translating these into practice and communicating to relevant teams.
Job responsibilities
* Lead and manage Tissue Viability Nurses, supporting their development, setting clear objectives, and fostering a positive learning culture.
* Lead Trust‑wide pressure ulcer reduction campaigns and embed sustainable improvements.
* Facilitate change management, lead local Tissue Viability projects, and provide expert guidance during After Action Reviews following hospital‑acquired pressure ulcers.
* Collaborate with the Clinical Audit Department to design, coordinate, and analyse Tissue Viability audits, producing SMART action plans and meaningful reports.
* Maintain accurate data systems, clinical records, referral databases and review policies such as the Pressure Ulcer Policy and wound care formulary.
* Participate in research, quality improvement initiatives, policy development, and introduce quality systems to improve safety.
* Ensure appropriate incident reporting, investigation and documentation, contributing to continuous learning.
* Communicate effectively with multidisciplinary teams, infection control, procurement, safeguarding teams, and external partners.
* Ensure mental capacity assessments related to Tissue Viability care are appropriately documented.
* Adopt a cost‑effective approach in all aspects of service delivery, balancing quality care with responsible resource management.
Responsibilities and key result areas to include
* Develop, implement and maintain tools for monitoring the prevalence and incidence of pressure ulcers.
* Provide a comprehensive, evidence‑based Tissue Viability service across the Trust, ensuring the provision of policies, procedures and information resources to support frontline staff.
* Operate a responsive referral service for complex and non‑healing wounds and pressure damage.
* Provide specialist assessment to patients who have complex or non‑healing wounds of all aetiologies and enable the provision of an effective, high quality, evidence‑based Tissue Viability service.
* Facilitate the implementation and sustainability of new Tissue Viability systems/processes at ward level.
* Integrate Tissue Viability theory into practice through interactive workplace learning.
Person Specification
* GCSE/'O' level English and Mathematics
* NMC registration
* Principles of Wound Care Module
* Experience of using a spreadsheet software, Microsoft Office and devising databases
* Previous involvement with data collection systems
* Awareness of risks, identifying them and alerting appropriate personnel
* Ability to work without direct supervision, aware of boundaries and need to seek advice and refer as appropriate
* Previous experience of supporting patient/clients
* Previous experience of participation in audit
* Awareness of changing agenda within NHS
