Key Responsibilities
* Provide expert clinical support and leadership to community nursing staff working within the Berkshire Out of Hours service.
* Collaborate closely with the Operational Out of Hours Team Lead to ensure staff competency, including overseeing induction processes and ongoing clinical development.
* Champion evidence-based practice to ensure delivery of high-quality, safe, and effective patient care.
* Act as a senior clinical resource, offering enhanced support and guidance for complex patient cases during out-of-hours periods.
* Promote and support initiatives aimed at reducing inappropriate or unscheduled hospital admissions and facilitating timely hospital discharges.
Qualifications & Experience
* Specialist Community Nursing Qualification (DN SPQ).
* Advanced History Taking qualification.
* Independent Prescribing qualification (V300) or nearing completion.
Clinical Governance & Care Quality
* Ensure care delivery is based on sound clinical assessment and robust care planning processes.
* Monitor and review staff practice regularly to maintain high standards of clinical care.
* Investigate complaints, safeguarding concerns, and serious incidents, identifying root causes and implementing improvements.
* Add areas of concern to the risk register and disseminate learning across the team.
Leadership & Team Management
* Exercise leadership and sound judgment in supporting the management of the Out of Hours Community Nursing Team.
* Support recruitment and retention strategies to build a resilient and skilled workforce.
* Lead on staff induction, personal development, and professional growth.
* Monitor and support clinical supervision, preceptorship, and competency development.
Training & Education
* Ensure all team members complete required statutory and mandatory training.
* Provide education and training opportunities for staff and students, including pre- and post-registration nursing students.
* Develop community nursing practice in a defined area through collaboration with primary care team members.
