Neighbourhood Transformation Manager

Brent Integrated Care Partnership (Brent ICP) brings together health and care organisations from across the borough to deliver services for the community.

Main duties of the job

The purpose of this post is to tackle health inequalities through a neighbourhood health and wellbeing approach, by leading the design and delivery of transformation projects across health, social care, wellbeing services and the voluntary, community, faith and social enterprise (VCFSE) sector.

1. Lead and take accountability for the delivery of projects that tackle health inequalities and deliver service improvement across the NHS, council and neighbourhood and place‑based partnerships; this will include, for example, areas of partnership collaboration (e.g. health inequalities, priority cohorts, system enablers) and service‑specific areas of focus within the neighbourhood team.
2. Lead the production of programme documentation, including programme plans, reporting templates and completing returns and reports as required.
3. Lead on relationship working with local residents on what is important to them and support aligning of services to support this way of working.
4. Represent NHS and council at external partner and stakeholder meetings, and substitute for the Director or others at senior partner meetings where required.
5. Embed workforce change into all improvement initiatives, supporting managers to identify and source appropriate OD & LD interventions.
6. Develop and implement practical ways of improving the quality and speed of communications from the programme to providers, staff and wider partners.
7. Develop local action plans in line with population health data and local insight.
8. Lead on relationship working with local residents and facilitate change in delivery of statutory services in response to local needs.
9. Support in empowering local community and faith‑based organisations to develop wellbeing for their residents.
10. Embed a consistent approach to tackling health inequalities in the neighbourhood, promoting community engagement and partnership working.
11. Keep up to date with major developments and learning within health and the council (especially adult social care, public health, housing, community development) to ensure the changes within services will produce improved user experience and outcomes.
12. Ensure current understanding of the main business processes, procedures and systems that support the relevant sector to provide high‑quality advice to operational managers on methods and approaches to make improvements.
13. Attend the relevant meetings and work collaboratively to identify current and emerging performance and practice issues.
14. Line‑manage programme officer(s) or interim staff, overseeing their work and delivery of their key programme objectives.
15. Actively engage with all internal and external stakeholders, developing and maintaining effective and strategic links; influence, challenge and empower managers and staff to change and improve their services.
16. Model a culture of collaborative working, setting high professional standards for the delivery of service improvement support to operational areas.
17. This is a role across health, care and wellbeing hosted by the Brent partners and the post holder will maintain integrity and commitment to both organisations’ values and objectives.
18. Develop and maintain effective relationships with Brent NHS providers, the voluntary, community sector and primary care and private sector providers.
19. Manage and lead staff to achieve high performance and effective operational delivery, including developing and improving staff capability.
20. Support effective partnership working relationships and act as an ambassador and advocate with external organisations.
21. Keep up to date with developments in service delivery and best practice to ensure the service performs effectively and to the highest standards.
22. Safeguarding is everyone’s responsibility and all employees are required to act in such a way that at all times safeguards the health and well‑being of children and vulnerable adults.


Person Specification


Education and Qualifications

* Educated to a related social care or health degree, or demonstrable comparable experience in adult social care, health or transformation development work, with evidence of significant relevant continuing professional development.
* Knowledge of the specific area of neighbourhoods acquired through a degree or equivalent experience and training, plus further staff and project management knowledge to post‑graduate diploma level or equivalent.
* A certified project management qualification such as LEAN or PRINCE 2.


Experience

* Working collaboratively across organisational boundaries to drive sustainable continuous service improvement.
* Supporting effective delivery of service/business plans to meet performance targets, managing and mitigating risk.
* Developing practical, innovative and creative approaches to service improvement securing ownership and commitment to key objectives across professional staff teams and building capacity and capability to deliver.
* Working effectively with stakeholders and partners and building positive cross‑sector relationships in support of key objectives.


Skills and knowledge

* Significant knowledge of and/or experience in adult social care, public health or health; in particular care homes, community health services, public health.
* Communicating and influencing skills.
* Contribute to the longer‑term development of the service area.
* Strategic thinking, analytical ability and a creative, entrepreneurial approach to managing complex issues.
* Communication and influencing skills to inform, consult and negotiate with a range of audiences in a straightforward, articulate, sensitive and persuasive manner, both orally and in writing.
* Interpersonal ability to engage with service areas, elected members, partners, stakeholders, the media and the wider community to engender confidence and respect.
* Organisational skills to plan and prioritise performance and improvement work programmes and multiple projects to deliver results on time and within budget, managing risk and resolving barriers to success.
* Identify and encourage innovative solutions, support a “can‑do” culture and achieve results.
* Effective leadership and co‑ordination of working groups and the achievement of positive outcomes.
* Show skills, abilities and expertise to work across health and local authority and build effective relationships in a complex and challenging environment.

Salary

£55,524 to £62,652 a year (inc HCAS, pro rata if part‑time).
