Responsibilities
* Collaborate with healthcare professionals to develop and implement discharge plans that meet the needs of adults transitioning from hospital to home.
* Conduct comprehensive assessments to determine appropriate support and resources for patients and their families.
* Provide timely and effective interventions to assist in the smooth discharge process.
* Facilitate communication between hospitals, patients, families, and community services to ensure continuity of care.
* Maintain accurate documentation and records in compliance with Social Work England standards.
Requirements
* Must be registered with Social Work England and possess a degree or equivalent in Social Work.
* Must have UK Qualified experience working as a Social Worker, ideally in a hospital setting.
