About the Role: Adults Social Care - Social Worker (Qualified) As an Adults Social Care - Social Worker, you will play a crucial role in facilitating hospital discharges efficiently. Your primary responsibilities will include ensuring timely reviews within two weeks in line with our new winter initiative, which emphasizes a home-first approach for service users. The aim is to support individuals in returning home safely and promptly, or conducting regular reviews for those unable to do so at the point of discharge.
