The flow co-ordinator role in mental health will encompass that of expert clinician and decision maker, educator, and leader. It will be a pivotal role to assist, support and promote a seamless patient journey up to and including discharge, promoting a positive patient experience. The caseload will vary daily and cover all East Lothian mental health patients in any acute setting, including REH and SJH inpatients, as well as admissions to RIE and WGH. The postholder will rapidly analyse assessments pertinent to each patient and synthesise a cohesive discharge plan. This may include, but is not limited to, Psychiatry, Nursing, Occupational Therapy, and Adult Social Work assessments.
Responsibilities
* Provide senior clinical support to the Service Manager through day-to-day co-ordination of the Mental Health Team with focus on patient flow.
* Offer highly specialist advice to healthcare professionals (IHTT, CMHT, CWIC, PTS, Psychiatry), other agencies, carers, clients and relatives.
* Support the Clinical Nurse Manager in the enhancement and strategic development of the existing service and any new services based on local community needs.
* Deputise into a leadership and management role in the absence of Team Lead.
* Work with clinical teams to develop and deliver programmes of care, including assessment, planning, implementation and ongoing evaluation of care needs for patients primarily in the domiciliary setting within East Lothian.
* Collaborate with Primary and Secondary Care Teams, Unscheduled Care Service and other agencies involved in community care to deliver services.
* Act as the key clinical lead with health and local authority colleagues to prevent hospital admissions and ensure early and effective hospital discharge; lead and develop multi‑agency teams to improve practice, implementing and evaluating evidence‑based standards, guidelines and policies.
Qualifications and Requirements
* Relevant professional qualification and registration as required by role.
* Experience in mental health settings and discharge planning.
* Ability to quickly analyse assessments and synthesise discharge plans across disciplines.
* Strong leadership and collaboration skills across health and social care partners.
