Role Summary:
Responsible for delivering high-quality care aligned with the home's philosophy and Care Quality Commission standards. Approximately 50% of the role involves direct care.
Hours: 7.4 hours per day 5 days per week.
Key Responsibilities:
* Manage and deploy staff effectively during shifts.
* Provide supervision and on-the-job training.
* Ensure care meets dignity and quality standards.
* Undertake direct work with service users and maintain person-centred plans.
* Monitor and review care management procedures.
* Maintain health and safety and safeguarding standards.
* Promote a customer-focused, flexible service.
Job Specifics:
* Qualifications: NVQ Level 4 in Adult Care or equivalent
* DBS Level: Enhanced
* Working Hours: Includes occasional evening and out-of-hours work
* Contractual Overtime / Standby Rota: Not applicable
* Politically Restricted: No
Key Duties:
* Manage caseloads and conduct needs assessments.
* Act as key worker for service users.
* Develop and review care/support plans and risk assessments.
* Promote self-advocacy and informed decision-making.
* Maintain accurate records using IT systems.
* Work across various settings (e.g., outreach, residential).
* Collaborate with agencies for multi-agency support.
Person Profile:Knowledge & Experience:
* Experience with adults/children/families, including those resistant to engagement.
* Familiarity with multi-agency work and care planning.
* Understanding of mental health, dementia, and challenging behaviours.
* Caseload management and ICT proficiency.
Qualifications:
* Level 6 (e.g., Bachelor's) or Level 5 (e.g., HND, Foundation Degree) or equivalent experience.
