Eliburn Support Service is a day service supporting adults with learning disabilities, physical disabilities, complex health care needs, or life-limiting conditions.
This is a fixed-term opportunity covering maternity leave, offering a chance to join our dedicated team.
The service provides face-to-face support with high staffing levels, enabling individuals to access meaningful activities in both building and community settings, tailored to their outcomes.
The role involves developing and delivering a person-centred, outcome-focused service in line with West Lothian Council policies, Care Inspectorate guidance, and national strategies.
The successful candidate will demonstrate practical skills, knowledge, and understanding in supporting individuals through a holistic, creative approach to meet social, health, and wellbeing needs.
Responsibilities include providing hands-on care, supporting personal care, physical mobility, nutrition, and hydration.
Supporting communication—both understanding and expression—is essential, helping individuals express their preferences and wishes.
This role can be physically demanding, requiring support for community and building-based activities, ensuring meaningful experiences for service users.
You will be expected to record information and maintain care plans, both electronically and in writing.
