An exciting opportunity has arisen for the employment of a general practice admin assistant to join our established team.
West Wakefield is a Primary Care Network (PCN) with a patient population of circa 68,000 across 5 practices.

We are looking to expand our GPA Admin team to work alongside our practices and other PCN staff.
Our PCN staff team currently includes Clinical Pharmacists, Pharmacy Technicians, Social Prescribers, Health and Wellbeing Coaches, Care Coordinators, Occupational Therapists, Mental Health Nurses, General Practice Assistants (clinical), General Practitioners and First Contact Practitioners.

Main duties of the job
KEY WORKING RELATIONSHIPS
Internal
Close working relationships with all members of the Conexus Healthcare team, the Chair of the Board, the Board of Directors and staff in our member practices & primary care networks.
External
Excellent working relationships required with Conexus Healthcare partners and shareholders.
ROLE SPECIFIC RESPONSIBILITIES
The GPA Admin Team are responsible for coding clinical records from letters and communications from external providers. In addition to inputting clinical data.
We welcome applications from both experienced clinical coders and those willing to learn and take on a new challenge. Training will be provided.
The GPA Admin team plan to develop the service offer as the team expands.
This is likely to include;
* Scanning and matching letters to patient records
* Sorting clinical post and prioritising actions for GPs and other PCN teams.
* Identifying clinical actions and directing to appropriate practice and PCN teams.
* Arranging Appointments, referrals, tests and follow-ups
* Managing caseloads & incoming referrals
* Identifying and acting on safeguarding issues.
* Clinical reporting and searches
* Summarising medical records
* Support the PCN and Practice with administration support as required.

This role requires the successful applicant to complete an enhanced DBS check.

Person Specification
Knowledge
* Awareness of the PCN DES contract
* Understanding of the importance of patient confidentiality
* Able to recognise the importance of accuracy and timely input into the patient record.
* Medical terminology
* Anatomical terminology
* Information Governance and Safeguarding
Other
* Able to meet Conexus Healthcare standards of attendance.
* Able to travel between sites and other venues within an appropriate timescale as required.

Qualifications
* Educated to GCSE level or equivalent
* GCSE Mathematics and English Grade C or above (or equivalent)
Experience
* Working as a part of a team.
* Experience working in a primary care/ general practice admin team.
* Experienced Clinical Coder/summariser
* Experience of SystmOne
* Experience of working in a PCN.
Skills
* Ability to communicate effectively both verbally and in writing.
