NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

The shift pattern for this post is: 4 days over 7, between 8am and 8pm, 365 days per year.

Are you passionate about delivering high-quality person-centred care in innovative settings? East Dunbartonshire is launching a new Hospital at Home Service, and we are seeking enthusiastic and dedicated Advanced Nurse Practitioners/aspiring motivated Trainee Advanced Nurse Practitioners to join our values driven multidisciplinary team. As part of NHSGGCs transforming together agenda, our Hospital at Home service will provide hospital level care to patients living with frailty in the comfort of their own home, including Care Homes. This new service will contribute to reducing unnecessary hospital admissions and supporting early discharge. This is a unique and exciting opportunity to be at the forefront of transforming healthcare services in East Dunbartonshire, enhancing outcomes for patients across the Health and Social Care Partnership, and contributing to service development.

The post holders will be required to use advanced assessment, reasoning, interpretation and decision-making skills to reach a working diagnosis, whilst working collaboratively with the responsible medical officer, to plan and deliver appropriate treatments. This will include accessing hospital level diagnostics and completing necessary hospital level interventions at home. The post holders will be key to continuous assessment and decision making as part of the wider MDT. They will lead discharge planning and will link with our local community services to ensure a collaborative approach to care during admission to hospital at home and to ensure a safe discharge.

This post is ideally suited to qualified Advanced Nurse Practitioners or individuals who are passionate about undertaking outstanding academic and final sign-off requirements for advanced practice and entry on to the SWISS register. Applicants who are applying for a trainee post are required to have experience at practitioner level and attained a minimum of two modules from the Advanced Practice programme. Qualified ANP applicants should demonstrate competence in the following areas:

Clinical assessment and diagnostic reasoning, clinical judgement and decision-making, anatomy and pathophysiology, non-medical prescribing (V300), leadership in care delivery and evaluation, practice-based learning or equivalent transferable modules.

East Dunbartonshire is proud to offer supportive and collaborative ANP led services within urgent and unscheduled care services. The successful applicant will have the opportunity to engage in structured CPD sessions and will be fully supported to address any ongoing training and development needs upon appointment.

As from 1st April 2026, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
