Operating across Walsall, Sandwell, Dudley, and Wolverhampton, the service provides consistent and effective support from hospital admission through to discharge.As a Senior Support Worker, you will play a key role in supporting the Service Manager in the day-to-day running of the Hospital Discharge Service. You will act as a deputy when required and serve as a central point of contact for the team, external professionals, and commissioners.Working closely with support workers, you will review ongoing cases and provide guidance on interventions for individuals facing mental health challenges or barriers to accessing services.
