Associate Practitioner

An exciting position has become available for a Band 4 Associate Practitioner to join the Mental Health Team in HMP Deerbolt.

This post is not open to trainees therefore the successful individual will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited into this Band 4 position.

You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.


Main duties of the job

This role is to support the planned care pathway within the team.

The successful candidate will have an opportunity to be involved in group delivery and coordination.

The successful candidate will support the physical care pathway within the team and will need to complete physical health checks for people on caseload, including ECGS, venepuncture etc.

We will provide you with clinical caseload and managerial supervision, a thorough induction and an extensive range of training opportunities relevant to the role.

Job responsibilities

To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.

To be compassionate in meeting the needs of patients and their carers.

To work as part of a community team which may be integrated, working collaboratively and in partnership with local authority services.

To deliver patient care, as directed by registered practitioners and the team manager, undertaking a range of clinical/therapeutic interventions with limited or indirect supervision in patients homes and other community settings.

To promote social inclusion, community access and participation through the provision of patient care.

To show commitment to and participate in quality improvement activities.

To promote at all times a positive image of people with mental ill health.

To promote at all times a positive image of Health and Justice services and the wider Trust.

Person Specification


Qualifications

* Foundation Degree in Health and Social Care or equivalent qualification such as Registered General Nurse
* Key skills in literacy, numeracy and ITQ level 2 (or equivalent)


Experience

* Previous experience of working with people with mental/social health needs or learning disabilities
* Previous experience as a Band 2/3 HCA or equivalent in a care environment.


Knowledge

* Person centred care
* Evidence based knowledge of current practices in the field.
* Awareness of legal frameworks including Mental Health legislation


Skills

* Effective verbal and written communication skills
* Time management and ability to prioritise effectively
* Be trained in Venepuncture


Personal Attributes

* Compassionate in meeting the needs of vulnerable people and their families and carers.
* Able to work flexibly and co-operatively as part of a team
* Committed to continual quality and service improvement
* Self awareness and committed to continual personal development.


Other requirements

* Ability to travel independently in accordance with Trust policies and service need.


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£28,392 to £31,157 a year (pro rata) per annum
