Patient Safety Incident Investigator

Eligibility: Current employees of the following NHS organisations:

* Cornwall Foundation Trust
* Royal Cornwall Hospitals NHS Trust
* Cornwall and Isles of Scilly Integrated Care Board

The post holder will carry out the investigation of patient safety incidents in line with the Trusts patient safety incident response plan, as well as complaints and other contained pieces of work (e.g. audits) as required, with a high degree of independence and autonomy. The post holder will be accountable for ensuring that the investigations are undertaken to a high standard and are completed within the required timeframes. The outcome of each investigation will include a report containing proportionate and measurable recommendations, which will be prepared by the investigator and agreed by a senior manager. An excellent command of written English will be essential.


Main duties of the job

Investigations include the requirement to interview a range of people involved with the incident. The patient, their families and carers are key stakeholders. Investigators will be required to demonstrate the ability to manage each scenario using facilitation and negotiation skills.

The post holder will be required to provide sensitive feedback to families, individuals and teams, as a result of the findings of an investigation.

The post holder will support the Senior Patient Safety Incident Investigator, the Patient Safety Specialist and the Head of Risk & Safety with other relevant tasks as required.

Person Specification


Education / Qualifications

* Significant experience of working within the NHS as a clinical professional or in an allied field public sector or safety-related field, including social care.
* Educated to degree / equivalent qualification & experience
* Training and experience in systemic incident investigation methodologies
* Experience of receiving, analysing and presenting highly complex sensitive data and information to individuals and large groups in a way that would be clear to those attending
* Experience of working across organisational boundaries within NHS and wider health/social care communities


Skills and aptitude

* Excellent written communication with extensive command of English, including excellent grammar and ability to communicate concisely.
* Ability to analyse complex facts or situations, where accuracy is important, in a logical and internally-consistent manner according to the requirements of each investigation
