We are offering an exciting opportunity to join our Sound Futures, Supported Housing Accommodation Service in the Plymouth area as a Housing Support Worker.
Provide tenancy-related support both in supported housing and through community outreach.
Collaborate with Plymouth Alliance partners, landlords, and housing providers to help customers transition into independent living.
Support individuals with a range of needs including mental health, substance use, budgeting, benefits, and employment.
Develop, review, and update support plans, risk assessments, and needs assessments.
Liaise with key agencies such as Probation, Police, Local Authority Services, and healthcare providers.
Promote social inclusion, wellbeing, and access to fitness and community activities.
Attend multi-agency meetings to coordinate effective support and risk management.
This role can be challenging at times, as you will be supporting individuals with low to medium complex needs from diverse backgrounds. At LiveWest, we believe in building brighter futures. This role is being offered on a full time, permanent basis working 37 hours per week. This is a scheme based role in Plymouth and you will be working on a 6-week rolling rota on a shift basis, working between the hours of 8am and 10pm, including some weekends.
We are committed to safeguarding and promoting the welfare of vulnerable groups. Appointment to this role is subject to a satisfactory enhanced DBS check.
To be considered for the role of Support Worker, you will have the essential skills, knowledge and experience for a Level 1 role (please see our candidate information pack) and the following role specific experience:
Have an understanding of support needs and support planning.
Have experience of risk assessing and risk management.
Have accurate record keeping and computer skills.
Have a flexible approach to work.
Have knowledge and understanding of welfare and housing benefits.
Have a good understanding of safeguarding and emotional and mental health issues.
