A new and exciting opportunity has arisen to join our friendly and supportive Mental Health Social Care Teams and support hospital discharge within Mental Health Services for the people of Derby City.
We are looking to appoint a Qualified and Experienced Social Work Practitioner with sound assessment skills, legal literacy and a demonstrable commitment to placing customers and carers at the heart of assessment and support planning. There is a strong culture of preventative, strengths-based and asset-based approaches within our work. We work closely with partner agencies to provide an integrated approach to the delivery of support. You should have experience of working with adults with severe mental ill health and of multidisciplinary working within a statutory framework.
These posts designed to improve collaboration at system level and improve working practices in relation to access to timely discharge from Mental Health Inpatient settings for Adults of all ages, and support a home first and recovery approach. You will be expected to provide high quality supervision to other social workers and Enablement Workers, as well as champion strengths and assets based practice. You will be an integral part of multi-disciplinary working, as well as leading on practice development in collaboration with NHS and Voluntary, Community and Social Enterprise Sector partners.
This post is a designated Approved Mental Health Professional (AMHP) Post. Therefore, you are practising AMHP or BIA this is advantageous.
