Job Responsibilities
In this role, you will deliver high-quality patient care by conducting clinical assessments, providing both telephone and face-to-face structured medication reviews, and managing patients with complex polypharmacy, particularly elderly individuals and those with multiple comorbidities. You will apply your pharmaceutical expertise to develop personalised care plans, support medication adherence, and promote safe prescribing practices. You will work closely with GPs, nurses, and other healthcare professionals to strengthen patient care pathways and ensure seamless transitions of care. The majority of work for this role is centred around care homes.
You will also play a central role in chronic disease management, overseeing long-term conditions and optimising treatment for patients with hypertension, lipid disorders, and other health needs. You will contribute to education and support by delivering patient education, training colleagues, and engaging with clinical research and innovation. You will help drive operational efficiency by managing prescription processes, responding to medication-related inquiries, and supporting quality improvement initiatives such as QIPP, QOF, and other financial initiatives offered by commissioning bodies.
Qualifications and Experience
Essential
* Undergraduate degree in Pharmacy with registration by the General Pharmaceutical Council (GPhC).
Desirable
* Postgraduate qualifications, such as a Masters in Pharmacy (MPharm).
* Independent prescribing qualification (or completion of the CPPE pathway).
* Completion of the Primary Care Pharmacy Education Pathway (PCPEP).
Experience
* Essential: Experience managing acute and long-term conditions commonly encountered in general practice. Strong problem-solving abilities with an understanding of when to escalate to senior professionals.
* Desirable: Minimum of two years experience as a clinical pharmacist.
