Overview
General Practice at Front Door (GPFD) Service across multiple locations in Norfolk & Waveney. The role is held by Norfolk Primary Care CIC (NPC), a Community Interest Company providing primary care services in collaboration with local GP practices. The GPFD Service streams patients away from the front door into a service capable of supporting demand, reducing avoidable ED attendances, and directing patients to appropriate services.
Job Responsibilities
* Identify patients appropriate to the service based on initial conversation and excellent communication skills.
* Clinically examine and assess patient needs from physiological and psychological perspectives and plan clinical care accordingly.
* Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with simple or complex, on-the-day needs.
* Proactively identify, diagnose and develop treatment plans for patients at risk of developing long-term conditions, as appropriate.
* Refer patients for investigations for specialist opinion, where appropriate.
* Diagnose and manage both acute and chronic conditions with an acute presentation, integrating drug and non-drug treatments into a management plan.
* Support patient adherence to prescribed treatments and provide continuity of care with appropriate follow-up.
* Take guidance from the registered GP when required and escalate to Acute Hospital staff as needed.
* Prescribe safely within legislative framework and local formularies; provide information on medications, side-effects and interactions.
* Undertake general telephone triage / first contact assessment as part of GPFD operations.
* Work within professional boundaries to ensure safe, timely patient journeys in conjunction with ED and Acute Hospitals.
* Maintain safe clinical decision-making and demonstrate critical thinking in the clinical process.
* Adhere to NMC / HCPC Code of Professional Conduct at all times.
Qualifications and Experience
* Essential: Registered nurse (First level) with BSc (Hons) or MSc Advanced Practice (or HCPC registration as appropriate).
* Post-registration qualification in Advanced Nursing Practice, autonomous practice or equivalent.
* Independent Non-Medical Prescriber or near completion.
* Basic Life Support.
* Experience as a senior clinician and in Primary Care settings.
Desirable
* Masters-level qualification; teaching/mentorship experience; clinical supervision training.
* Experience in leading/managing clinical teams and quality improvement; experience in unscheduled care services.
Disclosures and Registrations
* This post requires current UK professional registration. Disclosure and Barring Service (DBS) checks are required in line with the Rehabilitation of Offenders Act. Applicants will need to complete a DBS submission.

* Contract: Permanent; Working pattern: Full-time or Part-time.
* Salary: Dependent on experience and qualifications.
* Locations include: 3rd Floor, Reed House, Peachman Way Broadland Business Park, Norwich; James Paget Hospital; Norfolk & Norwich University Hospital; Queen Elizabeth Hospital; Colney; Other local sites as required.
