The Role:
A vacancy has become available for a Physiotherapist within the East Dunbartonshire Community Rehab Team. The multidisciplinary team works with individuals in their home environment to identify their goals and priorities and support improvements in independence, function and mobility. Patients present with a wide range of conditions, from newly diagnosed illnesses to end‑stage palliative care. This post sits within the Care About Physical Activity (CAPA) team, providing physiotherapy input to individuals living in care homes throughout East Dunbartonshire. The role includes core rehabilitation, targeted projects with local care homes in line with the CAPA programme, and falls education, training and assessments. The post holder will work as an autonomous clinician within the speciality, taking full responsibility for an identified caseload, including assessment and treatment planning as required.
Duration, Location, and Working Pattern:
* Permanent, Part‑time
* Base location – Kirkintilloch Health and Care Centre
* Work is carried out across care homes and patients’ home environments throughout East Dunbartonshire
Key Responsibilities:
* Deliver physiotherapy assessment and treatment within the community and care home settings
* Support individuals to improve independence, mobility and functional ability
* Manage an independent caseload autonomously within the speciality
* Deliver core rehabilitation interventions
* Lead and contribute to targeted CAPA projects within local care homes
* Provide falls education, training and assessments
* Work collaboratively as part of the wider Care Home Liaison Team
* Liaise with patients, care home staff, families, third sector agencies and independent sector agencies to ensure optimal care
* Participate in local audit and clinical effectiveness projects
* Support monitoring and evaluation of service provision alongside the Team Lead
Essential
* Degree in Physiotherapy or equivalent qualification
* Current HCPC registration
* Full driver’s licence
Desirable
* Post‑graduate experience
* Community physiotherapy experience
