We have an exciting role on offer within our Adult Social Care and Health Integration as a Customer Support and Advice Worker.

About ACT (Accessing Change Together)
ACT is an integrated service that brings together council staff and partner organisations to provide support and interventions for service users who need assistance with housing, homelessness, domestic abuse, and substance misuse. It uses a shared case load management system to enable easier access to services and better collaboration between teams.
Staff will work alongside Adult and Children’s Social Care, Public Health, and other services to develop consistent pathways, effective processes, and shared risk assessments and support plans.

Responsibilities
Provide customer support and advice to service users accessing ACT, supporting them with housing, homelessness, domestic abuse, and related needs.
Develop risk assessments and support plans in line with inter‑disciplinary team input.
Collaborate with multiple agencies and partners to ensure a coordinated approach to service delivery and information sharing.
Contribute to the development of effective pathways and consistent processes across the integrated service.
Qualifications & Experience
Experience working effectively with a range of services and support organisations.
Drive, passion, and resilience to work creatively with service users who need ongoing support in housing, homelessness, and domestic abuse.
Strong communication skills and the ability to work with a multi‑disciplinary team.
Working Arrangements
Flexible working with the option of a blended environment (home and office where appropriate).
