Main duties of the job
Clinical responsibilities:
* Safely manage undifferentiated and undiagnosed presentations using data gathering, assessing for red flags, investigation, diagnosis, management, and review within their agreed scope of practice and capabilities.
* Demonstrate complex clinical reasoning skills, using problem-solving approaches to analyse and synthesise information, manage medical and clinical uncertainty, including physical, mental, and social problems.
* Have a sound understanding of referral pathways, including routine, urgent, 2-week wait, and emergency admissions.
* Use resources appropriately and effectively.
* Manage acute/chronic presentations and medical complexity.
* Safely and effectively use a range of consultation modes, including telephone, video, online, and face-to-face.
* Prescribe treatment and medication safely in line with local and national guidelines (where professional regulation and training permits, i.e., an independent prescriber).
* Offer personalised care to people, signpost, and link with other members of the organisation or wider health and social care team.
* Provide accurate, contemporaneous, and complete records of patient consultations, consistent with legislation, policies, and procedures.
* Support the practice's on-call triage system.

Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is characterized by a high degree of autonomy and complex decision making, underpinned by a Masters level 7 award or equivalent encompassing clinical practice, leadership and management, education, and research, with demonstration of core capabilities and area-specific clinical competence.
The post holder must be a non-medical prescriber with a minor injury and illness qualification, able to maintain high standards of care.
Advanced clinical practice involves managing care in partnership with individuals, families, and carers, analyzing complex problems across various settings to improve outcomes.

Main Duties & Responsibilities:
* Work as part of a multidisciplinary team to provide healthcare services to patients.
* Operate as an autonomous practitioner, responsible for evidence-based practice within your scope.
* Develop a portfolio demonstrating competencies in clinical practice, leadership and management, education, and research.
* Demonstrate highly developed interpersonal and communication skills.
* Develop critical thinking, clinical reasoning, and problem-solving skills.
* Practice as an autonomous primary care healthcare practitioner without direct supervision.
* Manage undifferentiated and undiagnosed presentations safely, including data gathering, examinations, diagnosis, investigations, prescribing, referrals, and follow-up.
* Provide health promotion and patient education, supporting self-care and healthier lifestyles.
* Work collaboratively with the team, understanding colleagues' capabilities to promote appropriate care.
* Manage urgent situations with effective emergency care, including BLS, anaphylaxis management, and AED use.
* Maintain accurate patient records and support the practice's triage system.
* Manage own and others' pathology results and clinical documents following policies.
* Communicate effectively with patients requiring additional assistance, respecting diversity and promoting equality and safeguarding.
